Patient Information  

 

     Admissions


 

     Admissions are accepted 24 hours a day, 7 days a week in the Admissions Office located adjacent to the Emergency Room triage.  Please bring  your insurance card, social security card, driver's  license, and Advance Medical Directive, if applicable.

 

 

     Discharges


     Your attending physicians will decide when you are to be discharged from the hospital.  He will discuss this with you or your family and write a discharge order that will be placed on your chart.  At the time of your discharge, you will be given instructions for your post hospital care.  A nurse will assist with questions concerning your post hospital care and assist you to your vehicle.

 

  

     Payments


     Payments can be made at the Front Office during normal business hours.  If you need to speak with someone about a payment schedule, please contact our Financial Consultant at (601) 384-8108 or cindyl@fcmh.net.

       Medicaid/Chips


     Information regarding Medicaid and CHIPS are provided to you as a service of FCMH and the Mississippi State Department of Health.  For information regarding Medicaid or CHIPS, please contact Catherine Sims at (601) 384-8106 or cat@fcmh.net.

 

  

     Advanced Directive


 

     You have the right to have an Advance Medical Directive, such as a living will or health care proxy.  These documents express your choices about your future care or name someone to make decisions if you cannot speak for yourself.  If you are not able to make your own decisions, Mississippi law allows you, your guardian, your agent under medical power of attorney or a substitute decision maker to make your healthcare decisions.  In the absence of an Advance Medical Directive or guardian, Mississippi law allows a person of your choice to be the substitute decision maker.  This document must accompany you each time you are a patient at the facility.  If you need more information regarding Advanced Medical Directives, please contact our Admissions Department at (601) 384-8102.
